Data Mapped to Summative Evaluation Report

Evidence from forms in the evaluation process are mapped to specific report fields in the summative.  What evidence is mapped to a particular report field is explained below.

Post Conference Summary Comments: Pulls evidence from formal observation post conference field.

Standard 1: Professional Knowledge

Report: Identifies the evidence that has been entered at the indicator level.  The report provides a visual using different gradients of orange associated with the number of times an indicator was identified as a strength or area for growth.

Evidence: Pulls evidence from informal observations and formal observations at the indicator level only.

Standard 1 Artifacts: Pulls all artifacts from Standard 1 in the documentation log.

Standard 1 Evidence Summary: Pulls artifact summary/reflection from documentation logs, Standard 1 Evidence from formal observation, informal observations at the standard level only, and Standard 1 comments from EP Teacher Interim Report (if used)

Standard 2, Standard 3, and Standard 4 same as 1 but for corresponding standard information

Standard 5: Learning Environment

Report: Identifies the evidence that has been entered at the indicator level.  The report provides a visual using different gradients of orange associated with the number of times an indicator was identified as a strength or area for growth.

Evidence: Pulls from informal observation and formal observation at the indicator level only.

Standard 5 Artifacts: Pulls all artifacts from Standard 5 in the document log.

Standard 5 Evidence Summary: Pulls artifact summary/reflection from documentation logs, Standard 5 evidence from formal observations, informal observations at standard level only, teacher survey growth plan - Including the following questions: What did learners perceive as your major strengths? What did learners perceive as your major weaknesses? Based on this information, what are your strategies for professional growth?  Teacher survey analysis - Including the following questions: What did learners perceive as your major strengths? What did learners perceive as your major weaknesses? Based on your strategies for professional growth as a result of your first survey, did you make any changes?, What were the results of the change(s)? What will you do differently the next time you teach this course? Standard 5 comments from the interim report (if used)

For school administrators survey summary includes: What did your respondents perceive as your major strengths?  What did respondents perceive as your major weaknesses?  Based on this information, what are your strategies for professional growth?

Standard 6: Professionalism

Report: Identifies the evidence that has been entered at the indicator level.  The report provides a visual using different gradients of orange associated with the number of times an indicator was identified as a strength or area for growth.

Evidence: Pulls from informal observation and formal observation at the indicator level only.

Standard 6 Artifacts: Pulls all artifacts from Standard 6 documentation log.

Standard 6 Evidence Summary: Pulls artifact summary/reflection from documentation log, Standard 6 evidence from formal observation and informal observations at the standard level only, and Standard 6 comments from the interim report (if used), evidence of progress toward achieving professional practice goal, and professional practice goal statement.

For educational specialists evidence from survey growth plan and analysis includes: What did respondents perceive as your major strengths? What did respondents perceive as your major weaknesses? Based on your strategies for professional growth as a result of your first survey, did you make any changes? What will you do differently next time? 

SLO Section

SLO Summary: SLO Goal Statement, Evidence of Progress Toward Achieving Goal, Revised SLO with Rationale (if appropriate), Mid-Interval Status of Goal, Evidence of Goal Completion, End-of-Year Status of Goal, and Lessons Learned.

SLO Self-Score Report: Provides the rating given by the evaluatee.

Holistic SLO Score: Provides the rating given by the evaluator after reviewing all SLO data for the duration of the evaluation cycle.
