User and Administrator Information on Forms

The first section of each form pre-populates information based on setting in MLP-OASYS and actions that have been taken.

Name:  Name entered on the User’s Information profile.

Building:  Building entered on the User’s Information profile.  (Optional)

Grade:  Grade level user supports. Entered on the User’s Information profile. (Optional)

Assigned Administrator:  If user generated forms (i.e. professional goal setting, documentation log, or survey) and the form has been assigned to an administrator the administrator’s name will be included.  Additionally, if the form is an administrator generated form (i.e. observation, interim, summative, performance improvement) the administrator’s name will be included if the form has been assigned to the administrator or the administrator will be assigned when the form is submitted to the user.

Submitted by:  The administrator who clicks the submit button on administrator generated forms or the user who submits the form on user generated forms.

Acknowledged By: On administrator generated forms the user is required to acknowledge the form.  The user’s name will then appear.  On user generated forms the user is not required to acknowledge the form since the user provided the information.  Therefore, the “Acknowledged By” will record N/A.

Finalized By:  The administrator who “finalizes” the form.

Title:  Job title entered on the User’s Information profile.  (Rarely used)

Department:  Department entered on the User’s Information profile.  (Optional)

Evaluation Type:  Evaluation type assigned to user for the evaluation cycle identified on the profile.

Evaluation Cycle:  The evaluation cycle set when completing the “Set -Up procedures” at the beginning of a cycle.  The evaluation cycle of all users can be viewed in “User Management”.

Date Submitted:  When the form is submitted by user or administrator the system automatically stamps the submitted date and time.

Date Acknowledged:  When an administrator generated form is acknowledged by the user the system automatically stamps the acknowledged date and time.  User generated forms do not have a date and time as the forms are not acknowledged.

Date Finalized:  When an administrator finalizes a form, the form is automatically stamped with the date and time of the finalization.
